Paper Enrollment Information
Welcome to the Paper Enrollment Process

Step 1 - Complete Enrollment Packet:
The packet can be downloaded or parents and guardians can complete it at the student's attendance area school or at the Education Center, 520 NW Wall Street, Bend:
- Enrollment Packet:
- Dental Screening (for children 7 years of age or younger)
- Learn more information about transportation on the School Buses website.

Step 2 - School Visit:
Bring the following:
- Completed documents listed in Step 1
- Proof of address (ex: utility bill, rental agreement, drivers license... Tax documentation is not accepted for proof of residency.)
- Birth verification (kindergartners must be age 5 on or before September 1 the year your student starts kindergarten)
- Immunization records (ex: CIS form, certificate of immunizations...) Details here.
- Recent special education records (if applicable)
- Custody documents (if applicable)
Parents and guardians may also need to fill out other documents pertaining to the school.

Interested in Bend-La Pine Online?
Families interested in Bend-La Pine Schools Online school must have a current enrollment into Bend-La Pine Schools (see above). Your child's enrollment will default to your neighborhood school based upon your residential address.
Once enrollment has been submitted, then please complete a BLPO registration request form HERE.
