Enrollment Documents
Collect Student Information
- Proof of age document (birth certificate, passport, or similar document).
- Proof of residential address. Examples include: rental agreement, utility bill (dated within the last 30 days at the time of enrollment), or governmental assistance statement. (Tax documentation is not accepted.)
- Parent name, home address, and date must be visible.
- Immunization records.
- Examples: CIS form or certificate of immunizations. Please visit our Immunization website for detailed information.
- Recent special education records (if applicable).
- Custody documentation (if applicable).
- Dental Screening (for children 7 years of age or younger)
- Emergency contact information.
- Previous school's contact information (not required at the start of Kindergarten).
If enrolling via Online Enrollment, you can upload scanned versions of these documents. Otherwise, please deliver them to your attendance area school.
Parents and guardians may also need to fill out other documents pertaining to the school.

Paper Enrollment Packets
The enrollment packet can be downloaded or parents and guardians can complete and/or submit it to the student's attendance area school or at the Education Center, 520 NW Wall Street, Bend:
