Aflac is Bend-La Pine Schools' current Section 125 provider, allowing ALL employees the ability to set aside money from their paychecks, on a pre-tax basis, for dependent day care expenses and/or unreimbursed medical expenses.


Bend La Pine Schools and Aflac have partnered with Wage Works to take over the administration of your flexible benefits program.  Wage Works is a leading provider of tax advantaged programs.  This change does not, in any way, affect the insurance products or service you may currently have with Aflac.

Our benefit plan year runs 10/1/2012- 9/30/2013.

Health Care Flexible Spending Accounts (FSA)

A Health Care FSA can save you money by allowing you to set aside part of your pay, on a pre-tax basis, to reimburse yourself for eligible medical expenses such as co-payments, deductibles, prescriptions, and much more.  Expenses incurred for you, your spouse, and other qualifying individuals are eligible for reimbursement.

Bend-La Pine Schools currently allows each employee the ability to set aside up to $2,500 for a Health Care FSA.

There is a monthly service fee of $3.60 that will be withdrawn from your paycheck when enrolled for a Health Care or Dependent Day Care FSA. If you are enrolled in both, you will only be charged one $3.60 monthly fee. 

You CAN NOT be enrolled in a Health Care FSA if you also have a Health Savings Account (HSA).

Dependent Day Care Flexible Spending Accounts (FSA)

A Dependent Day Care FSA allows you to set aside pre-tax dollars to reimburse yourself for incurred eligible dependent care expenses. You may allocate up to $5,000 per tax year for reimbursement of dependent day care services ($2,500 if you are married and file a separate tax return).

There is a monthly service fee of $3.60 that will be withdrawn from your paycheck when enrolled for a Health Care or Dependent Day Care FSA. If you are enrolled in both, you will only be charged one $3.60 monthly fee. 

Important Reminders:

* Tuition-based Kindergarten through Bend-La Pine Schools is not an eligible daycare expense
* You may only be reimbursed up to the amount actually contributed to your Dependent Day Care account for the plan year. Eligible expenses in excess of this amount will be carried over and reimbursed as contributions accumulate.


 
 

 
Aflac WAGE WORKS

When you enroll in a Health FSA and/or a Dependent Care FSA (new this year) account you will be issued a Take Care benefits debit card.
 
With the Take Care card you can:  
  • access your FSA funds so you don't have to use your out of pocket dollars to pay for medical, dental or prescription expenses
  • reuse your card for future plan years so you don't have to throw it away once your funds have been used for the current plan year
  • the Take Care card is replacing the Aflac NOW card.  Aflac NOW cards are no longer active.
The Take Care card will be accepted at qualified doctor's office, pharmacy, dental office, vision care office and hospital etc. Please note that your card may not work at discount stores and grocery store pharmacies. Your Take Care card may be use at a dependent care provider if they utilize a credit/debit option for payment.
 
The Take Care card works like a debit card, requires no PIN and is active upon first use. Benefits are deducted from your FSA account immediately and the provider is paid immediately at time of purchase. The card does not eliminate the need for receipt substantiation. If Wage Works is unable to approve a transaction automatically as an eligible expense, Wage Works will notify you by email- or postal letter if a current email address is not on record- to submit verification of the expense.  If we request verification of a transaction, you will need to take action to resolve the matter.
