For Tier One and Tier Two members retiring within the year, this hour-long, live, interactive internet-based session provides details on which forms are required and the appropriate way to fill out the PERS service retirement application and the Individual Account Program forms.

* Opt Out Option for Classified Employees * Classified Employees may OPT out of their medical, dental and vision benefits and receive $225 / month. You must log into OEBB, and choose to OPT out of medical, and waive dental and vision coverage. You will need to provide proof of other group medical coverage while in the OEBB system. The $225 / month will be considered taxable income and added to your monthly paycheck.

Insurance Rates

2012-13 Employee Rate Sheet
|
|
Classified employee rates are based on a composite rate structure, meaning it is the same price to insure an individual as it is a family.
Rates will be deducted from an employee's paycheck on a pre-tax basis unless otherwise indicated by the employee. The premium dollar amounts listed on the rate sheet do not include any monthly deductions for Long Term Disability and Life Insurance. Monthly LTD rates through OEBB are based upon an employees salary. LTD and Life Insuarnce benefits paid to the employee are non-taxable, the insurance premiums for these coverages is deducted on an after-tax basis and is not subject to the Section 125 form. * OEBB plan year: Classified Employees may OPT out of their medical benefits and receive $225 / month. This amoung will be considered taxable income and will be added to your monthly paycheck.

If you have a stipend from the district you are using toward the cost of your retiree insurance, that stipend will be applied toward the monthly premium of your total coverages. The remaining dollar amount will be due to Bend-La Pine Schools on a monthly basis.

Voluntary Short Term Disability
Through Standard Insurance & OEBB, eligible employees can purchase a short term disability policy for themselves.
Plan Summary Rates Voluntary Life Insuarnce Through Standard Insurance & OEBB, eligible employees can purchase additional life insurance for themselves, a spouse and/or children. Plan Summary Rates Voluntary Accidental Death & Dismemberment Through Standard Insurance & OEBB, eligible employees can purchase additional AD&D coverage for themselves, a spouse and/or children. Plan Summary Rates Voluntary Long Term Care Through Unum & OEBB, eligible employees can purchase LTC insurance for themselves, a spouse and/or children. Retiree Information
|
Classified retirees have the same plan offerings as the active Classified employee group. Retirees do have the option of choosing each piece of their insurance coverage. Retirees must choose at least a medical plan, dental with orthodontia and vision are optional. * If a retiree chooses to opt out of medical coverage, proof of other group coverage must be provided to purchase a dental or vision plan with OEBB. * If a retiree chooses to not take any portion of coverage, the coverage may not be added back at a later time. * If a retiree chooses not to cover an eligible dependent, that dependent may not be added back at a later time.
* If you or a dependent are covered or become covered by Medicare, you CAN NOT continue to be covered by an OEBB Retiree insurance policy.
